We are currently recruiting a full-time Social Worker III to work for a medical center within the Rutland VT area. This Position offers a competitive salary and robust recruitment package. Summary
The Social Worker III is responsible for providing social work services to all customer groups affected by illness/ disability often including lifestyle adjustment difficulties. Understands utilization criteria and case management principles in a health care setting, as well as relevant community resources, insurance, regulatory, and legal system knowledge.
Conduct Social Work Assessment/Education services:
· Assesses, within the framework of a holistic approach, the social, emotional, familial and financial factors impacting the patient/family’s ability to cope with problems of daily living related to their mental health condition within, 48 hours of admission.
· Provide supportive education to individuals and families regarding such issues as: i.Alternate care placement.
· Financial/insurance planning.
· Resources for D/C related needs.
· Preventative and protective services for patients and caregivers.
· Crisis intervention.
· Create treatment goals utilizing results of individualized assessments.
· Coordinates inclusion input from patient support network in therapeutic plan
· Provide suicide risk assessments on psychiatric patients. Communicate findings to team, and recommend safety interventions based on findings.
· Drafting of and setting parameters in behavioral plans.
· Provide one on one therapeutic support.
Facilitates psycho-educational and other therapeutic groups.
· May Assist with safety checks and other patient care related functions including admission paperwork, searches, and other milieu management tasks as assigned
· Assess acuity and levels of care
· Other duties as assigned
Minimum Education
- Master's degree in Social Work (MSW).
Required Licenses/Certifications
- BLS Certification through American Heart Association*
- Completion of endorsed patient/visitor de-escalation and restraint program certification*
- Licensed in the State of Vermont as an LICSW. *Must obtain within 3 months of hire.
- 1+ full years of experience required
